@mansr Because the pilot needs to ensure the pad is clear before attempting to land, and many times that's difficult to see out the front of the aircraft, so a spiral is made so the pilot can look out their side window to better see the landing site.

@penguin42 @mansr I agree that waiting for the pad to clear is most likely explanation. More esoteric cases include things like being held up at an altitude by traffic control, and ending too high. A spiral is much safer way to lose altitude than a vertical descent, due to the hazard of so-called "vortex ring state". @penguin42 @avia It was (seemingly) a patient transport from one hospital to another. Depending on the urgency, noise levels might not have been the top priority.
