       after falling down a wikipedia hole last night, i found this in depth article from the 90s about how multiple programming errors led a radiation chemotherapy device to give lethal doses of radiation to some patients under certain circumstances. i figured some of the programmerfolk in particular on the fedi might find this interesting, although this nonprogrammer was also able to understand it.http://www.ccnr.org/fatal_dose.html
