       1) The Doctor Lobby needs to stop fighting to keep out foreign-trained physicians. Physicians from peer nations should be allowed to practice in the US after, say, 1-2 years of supervision under an American attending (mostly to allow time for foreign-trained physicians to adapt to the US system).2) The US needs to drastically increase the number of med school and residency spots, opening many new med schools if necessary.

       3) NP programs need to have strict requirements for years of clinical experience in specialty for all students.4) NPs and PAs should be required to practice under the supervision of a physician or a more senior NP or PA for several years before obtaining FPA.5) NP and PA supervision should be meaningful and not just consist of a mid-level being required to work for and give a percentage of their earnings to another provider.

       One issue I didn't address above is that many US physicians say they need to earn more than peers in other countries because US med school is so expensive. This is a valid concern. But it's also the end result of a vicious cycle that started with restricting med school and residency spots to boost physician incomes. Fewer med-school and residency spots mean more income for fewer physicians.

       I hope that At some point during oral arguments in the student loan case, somebody points out that unlike Brett Kavanaugh, most student loan borrowers can't just have their debts paid by mystery benefactors that are never disclosed to the public.

       The SVB bank run and the calls to make depositors whole beyond FDIC limits are a great reminder that capitalists who make money by owning things rather than making things (aka "passive income") claim they nevertheless deserve what they have because investing involves risk. However, when capitalist spin the wheel and lose, they expect the state and by extension the public to cover their losses.

       My entire adult life I've seen this type of "lemon socialism" over and over again. Privatize the gains, socialize the losses. But on the rare occasions when the government has enacted policies like student loan relief or pandemic stimulus that help anyone who isn't already rich, the right and the "moderates" and "centrists" who are their useful idiots howl about moral hazard and welfare hammocks and the evils of government "hand outs".

       @lowqualityfacts Won't somebody please think of the *synergy*. In-person collaboration is vital to success. How will we get anything done if employees don't have to constantly listen to racist jokes in the breakroom or fend off unwanted advances from supervisors.

       @laurenSpecifically, young people (at least American young people) don't spend a lot of time overseas backpacking, working abroad, etc. I've heard that one reason so many LDS members work in the IC is that their missionary work gives them excellent foreign language skills as well as time overseas where they can account for exactly where they were and what they were doing and have it corroborated by other people.

       @erinwhalen The biggest impediment to the widespread adoption of ebikes in the US, IMO, is the abject lack of protected bike lanes and other infrastructure. Until cyclists can get to where they are going safely and without fear that they will be injured or killed by a car, ebike adoption will be slow, particularly in car-dependent suburbs.

       @erinwhalen Taking space away from cars to create protected bike lanes is never popular with drivers, but I really wonder how long it will take for cities and counties in the US to figure out that creating and maintaining a robust system of protected bike  infrastructure makes a place a *very* desirable place to live.

       Mastodon's lack of search and quote boost features are directly related to the lack of journalists who have migrated here from Twitter. Journalists are producing content they want shared and discussed. But they don't necessarily want to engage in a conversation with the people doing that. This is exactly what quote tweets are good for. The kinds of conversations (good and bad) that happened between big name accounts on Twitter are also just not possible without a comprehensive search feature.

       Apparently Blue Sky has both of these features which probably means as soon as it opens up to everyone, people are going to leave for Mastodon for Blue Sky.        It infuriates me how stupid and weak Biden has made himself look by negotiating with McCarthy on the debt ceiling. McCarthy won't accept any deal that doesn't throw everyday Americans – including huge parts of Biden's coalition – under the bus.Meanwhile, negotiating with McCarthy legitimizes taking the American people and the American economy hostage as a bargaining tactic.

       All the major Baltimore area school districts are back in session this week (many went back last week) and the morning traffic is just awful – particularly on 695. Traffic feels like it is as bad or worse than before COVID. My intuition is that much of this is down to many employers – public and private – requiring employees to return to the office at least 2-3 days per week. What a colossal policy failure and clusterfuck.

       Despite what managers and executives say, there is no evidence that compelling workers to go to an office leads to more productivity or efficiency than allowing employees to work from home. Indeed, my anecdotal experience and that of most people I know is just the opposite: it is markedly more easy to get stuff done working from home than working in an office.

       My wife recently switched jobs. Her former job was 100% work from home while her new job allows only one full day or two half-days of WFH per week. One of the first things my wife said after starting her new position is that she forgot how difficult it is to concentrate and get things done in ab office due to the constant noise and interruptions. She feels like she gets substantially less done at her new job than. Her old one.
