       They take the money you pay and invest it. They generate profit from low risk sources. Annuities is a big example of that. They bet that you will die before they have to pay you more than you invested in them, and set rates accordingly. They take that money and they invest it into various things. They usually off shore the business (like to Bermuda) because they require insurance companies to have less cash on hand than the US, meaning they can invest more of it.

       None of that is why they get admitted to the nicu. The most common reason is preterm births. And that does not explain why it costs so much.
