Rheumatology already ran a ANA comprehensive panel that checked for Sjogren's syndrome but I'm not opposed to asking them for a secondary test for it. Most of the doctors are pretty willing to look at possibilities. I just got back my paraneoplastic antibody panel from Mayo which was negative. And this week I'm going in for a biopsy to check for small fiber neuropathy. Also brought up what a superchatter raised on one of the streams about Porphyria and they agreed that AIP wasn't a bad idea and was relatively underdiagnosed so they are running UAs to check that out this week as well. I'm hoping in the coming month genetics and immunology can pin it down more. If not I may just post my blood work and imaging results and let people have a crack at it. Tell Sue thanks for the suggestion.

Check out the image.When that happens your whole body can go haywire. Growths, blood pressure changes, metabolism and hunger, vision issues, pancreas going wild, and much more, including all those thyroid issues mentioned by the linked tweet. This doesn't mean it can't produce enough juice to get (some) stuff working sometimes, but you have a rocky road of all sorts of things going wrong.

Jim, as a FYI, Sjögren's is commonly diagnosed with other rheumatic illnesses so you should ask for all the tests they’ll give you. It’s all bloodwork so it would be easy to catch a diagnosis or at least rule out many others. There’s tons of tests and different doctors do different choices of testing. Your previous ANA test might have been a general assessment. There’s more specific Sjögren's antibody anti-SS-A and anti-SS-B and other antibody, inflammation marker, and genetic tests they can do for you. Also, Rheumatology is weird too because you have an illness but still test negative and just have to work off symptoms for treatment.

The ANA was a comprehensive one so it tested for the Sjogrens antibodies. I brought this up today when I went in to do the UA for porphyria if they thought any follow up testing outside of the bloodwork would be worth it and they are discussing having me get a lip biopsy to check.

There was a very specialized clinic dedicated to diagnosing hard to pin down cases here in Minnesota headed up by some really great doctors but its way outside of insurance and the cost just to walk in and sit down is in the thousands. I could only imagine what going abroad would entail and with them concerned that it's something like a completely broken immune system I'd have to weigh the cost benefit of even trying that.
