@issdeinschnitzel Makes complete sense. The donor and the recipient have to go into a hospital and various medical staff need to be exposed to their bodily fluids. Hospitals are the place wher eyou can't afford outbreaks. You can't compromise those resources for the sake of a single patient. The risks of donation and recipient are far far greater than the risk of a vaccine.

A hospital has to know they've taken every reasonable step to prevent an outbreak in a hospital. Telling the patient and donor to get vaccinated is one of those steps; no need to trust their testimony on wether they were infected or not.

However, the original paper I linked, which balances misses against false alarms, finds the statistically optimal cut-point to be 26 (at least concerning mortality). Obviously policy will be more conservative than this, but, a cutoff of 40 gives you a specificity of 0 in this case, or a 100% false-alarm rate, ct>38 gives you 90%-95%, also fairly useless.
