Changes for ansi/ape (v10)

Makefile: add #define _POSIX_SOURCE (and undef BSD, SYSV)
change Makefiles to use CC=/usr/ape/apebin/pcc

tex/extra.c mf/extra.c: define index, rindex, not sprintf
	also, give implementations for putw, getw

mf/MFwindow/x10.c: don't declare sprintf

mfware/mfwarext.c: include string.h, don't declare sprintf

fontutil/extra.c: define index, rindex

bibtex can't be made with ANSI web2c, because it assumes that
by declaring an arg to be pointer to int, and passing an int,
the compiler will do the coersion.
solution: change web2c/web2c.c to allow -a0 to turn OFF ansi
mode, and add -a0 to web2c call in bibtex/convert

web2c/web2c.lex: change conditions for declaring unput, input to

tex/extra.c: add SIGNAL_HANDLER_RETURN_TYPE to def of catchint

texware/dvitype.ch: changed type of cur_name and real_name_of_file
	from text_char to char
texware/pooltype.ch: changed type of pool_name to char

mfware/gftodvi.ch: changed UNIX_file_name, real_name_of_file to
	packed char arrays
mfware/mft.ch: changed mf_file_name (et al.) and real_name_of_file to
	packed char arrays
mfware/mfware.h: declare openf if ANSI

Changes for local needs

applied tex/bigtex.diff and mf/bigmf.diff patches

mf/MFwindow/x11.c,x11-Xlib.c,x11-Xt.c: surround X includes
	by undef of __STDC__ (a bug in need-prototypes?)
