App that I can use to collect Request details from Customers

Evening,

So I am trying to come up with a way my customers can use an online
form to fill in their details asn requests. I currently use an Excel
sheet .

We install Gas and Electricity meters on sites etc

Sheet 1 - Customer Details (Name, address Reg no etc)

Sheet 2 - Site details (Name address etc)

Sheet 3 - List of Electricity Meters needed and location can be
hundred +

Sheet 4 - List of Gas Meters needed

Sheet 5 - additional Info

Any ideas on selfhosted apps I could use to collate this from the user
rather than having to send the spreadsheet ?

Have you try Google forms?

If you want some more complete, check REI3, it’s selfhosted, and you
can draw your pages, códigos costumize workflow, login and user
management.

It’s has a base on its own, but is nice to add our workflow and pages.

You could also try out one of the general ticketing systems like
OsTicket, Zammad, etc. Then modify the entry forms.
