Hello all,

Not sure why I'm having such difficulty tracking something down, what
I'm looking for is a simple web based form that allows for digital
signatures. Essentially just a form that users can utilise to request
additional access to computer systems, which their supervisor can then
sign off and submit, with a saved copy of the transaction being kept
somewhere on disk.

There are two caveats, it needs to be entirely self hosted (ie. no
cloud components) and free (in particular, my budget is $0).

Has anyone got any recommendations?
