Fasten Health - An Open-Source Personal Medical Record Aggregator - v1.0.0 Roadmap - Desktop App, DB Encryption & Medicare/Kaiser/UHC Support! reddit / selfhosted ---------------------------------------------------------------------- analogj 路 2023-11-14 13:05 Hey reddit! We're getting ready to release v1.0.0 of [Fasten Health](https://github.com/fastenhealth/fasten-onprem)! > Just a refresher: [1 year ago](https://www.reddit.com/r/selfhosted/c omments/xj9rx7/introducing_fasten_a_selfhosted_personal/) I announced **Fasten, an open-source, self-hosted, personal/family electronic medical record aggregator (PHR), designed to integrate with 100,000's of insurances/hospitals & clinics**. > > It securely connects patient healthcare providers together, creating a personal health record that never leaves the patient's hands without their consent. > > Here's a couple of screenshots that'll remind you what it looks like: > > [Fasten Screenshots](https://imgur.com/a/vfgojBD) --- I'm thrilled to finally share the roadmap for our much-anticipated Fasten Health Version 1 release! 馃搵 **Key Features and Milestones for Version 1:** 1. **Database Encryption at Rest:** - Security is at the core of Fasten Health. In v1, we've implemented robust database encryption at rest, ensuring that your health data is stored with the highest level of protection -- protecting your data even as it lives on your own device. 1. **Desktop Apps for Mac (and Windows soon):** - The [Fasten Health for Mac Desktop App](https://forms.gle/eExjsMN912bXBZ538) is available as a Public Beta! Desktop apps open Fasten Health to a whole new world of users. No longer do you need to be a technologist who knows Docker inside out. This move not only helps us refine our product with diverse feedback but also contributes to the growth of our community. At the same time, it empowers patients by offering them a new app that solves real health-management related problems without compromising their data privacy. - [Click here to try it out](https://forms.gle/eExjsMN912bXBZ538) 1. **Manual Medical Record Entry:** - We understand the importance of flexibility in managing your health data. With our upcoming v1 release, you'll be able to manually enter important aspects of your medical history, including conditions, medications, procedures, lab results, private patient notes and more. This feature ensures that your health record is truly comprehensive and reflective of your unique health journey. 1. **Support for most Major Healthcare Providers in the US:** - We are delighted to announce that Fasten Health integrates with Medicare, United Healthcare, and Kaiser Permanente, among countless other major medical providers. This means that our platform now supports 40 of the 50 largest insurers in the US, along with a vast network of healthcare institutions (almost 28,000 at last count), offering you unparalleled access to your health data in one centralized hub. A heartfelt thanks go out to the numerous Github users who've contributed and the Reddit/Discord users who've discussed & brainstormed with me. As always, your trust and support has been incredibly humbling and keeps me motivated! Glad to see you getting close to a full 1.0 release :) ---------------------------------------------------------------------- analogj 路 2023-11-14 18:39 Thanks! It's been a long & complicated process, but I think we're building something that needs to exist in the world. ---------------------------------------------------------------------- FunnyPocketBook 路 2023-11-14 22:19 This looks amazing! Is there a list of all supported institutions? I don't want to install it and only then find out that my institution isn't supported. Also, I'm assuming this is US-only? Didn't see this mentioned anywhere, only tangentially on this Reddit post ---------------------------------------------------------------------- analogj 路 2023-11-14 22:29 > Is there a list of all supported institutions? You're right, I should make that available outside of the app somehow. The best way to see if your provider is supported is using this [Google Sheet](https://docs.google.com/spreadsheets/d/1ZSgwfd7kwxSnimk 4yofIFcR8ZMUls0zi9SZpRiOJBx0/edit?usp=sharing) - though its a bit out of date, showing 24,000 providers instead of the current 28,000. > Also, I'm assuming this is US-only? Theoretically Fasten can be used by any country that has a public patient-facing FHIR API, but I've only found them in the US & Australia (other countries are still in the testing/validation phase). We're tracking [international support here](https://github.com/fastenhealth/fasten-onprem/issues/42), but feel free to comment if you found information about your country! ---------------------------------------------------------------------- Gold_In_Kabul 路 2023-11-15 01:54 This is wild, and very important. ---------------------------------------------------------------------- OhMyForm 路 2023-11-15 14:21 Can there be a version of the world where you support Canadian healthcare too? ---------------------------------------------------------------------- analogj 路 2023-11-15 14:24 Thanks! ---------------------------------------------------------------------- analogj 路 2023-11-15 14:30 Yes, I鈥檇 love that because I鈥檓 Canadian too! However Canada doesn鈥檛 seem to have any public patient-access FHIR API endpoints, they seem to still be under development or restricted to internal testing. We're tracking international support on a github issue, and once Canadian APIs become available you鈥檒l definitely see them in Fasten ---------------------------------------------------------------------- chrishas35 路 2023-11-20 01:58 >Support for most Major Healthcare Providers in the US: We are delighted to announce that Fasten Health integrates with Medicare, United Healthcare, and Kaiser Permanente, among countless other major medical providers. The inclusion of United excited me, but apparently it does UHC does not support FHIR for their commercials plans ([\#318](https://github.com/fastenhealth/fasten-onprem/issues/318)). Bummer. ---------------------------------------------------------------------- gregspinks1987 路 2023-12-05 19:32 Any scope for NHS from the UK? ---------------------------------------------------------------------- analogj 路 2023-12-05 21:40 Yep, we're tracking [international support here](https://github.com/fastenhealth/fasten-onprem/issues/42). Unfortunately it seems the NHS patient-access API is still being tested. ---------------------------------------------------------------------- uninvitedguest 路 2023-12-24 03:51 This is really interesting. I pulled the current version and gave it a spin and... didn't find it a good fit for my use case. * It's bit overkill for a PHR database where I'd be entering all of the data manually (Canadian here, no integrations). * The manual entry workflow is extremely cumbersome and not very intuitive - there's no way that I could get my wife to make use this to record any illnesses at home (temperature readings, time of vomiting, for example). * None of the manually entered information can be edited, before saving an encounter or after. Information entered in an encounter (height, weight, temperature, etc) cannot be viewed in the medical history. * Widgets on the dashboard say they are missing data but don't hint as to what data they would display. * Last updated date is always Oct 10, 2018 I realize the software is pre-release though I'm not sure what version would have pulled from the main branch at this time. I would love to have a searchable timeline approach to recording/retrieving medical data, but pretty much all home care is going to be manual entry and the current manual entry flow is unusable. Will that see a big jump in v1? ---------------------------------------------------------------------- analogj 路 2023-12-25 17:10 Hey, Appreciate the feedback! > - It's bit overkill for a PHR database where I'd be entering all of the data manually (Canadian here, no integrations). > - The manual entry workflow is extremely cumbersome and not very intuitive - there's no way that I could get my wife to make use this to record any illnesses at home (temperature readings, time of vomiting, for example). Agreed, Fasten is initially designed for pulling health records automatically. The manual data entry flow is something that I've added after the fact, and it's grown organically. It also doesn't help that I'm an infrastructure engineer, not a frontend engineer, so my UX skills are pretty bad :( I'm definitely open to ideas and feedback here: - **What would make the manual data entry flow clearer?** - **Can you share links to other apps/screenshots that allow for manual entry in a simpler/more intutitive way?** > - None of the manually entered information can be edited, before saving an encounter or after. Information entered in an encounter (height, weight, temperature, etc) cannot be viewed in the medical history. So this is related to Fasten's origin, it pulls records from EHR systems, but it's not the source of truth for any of that data. I made a conscious decision not to allow for editing of sync'd data because I didn't want to confuse users. However, as you've pointed out, data entered manually should definitely be editable. We'll add some editor functionality eventually. > - Widgets on the dashboard say they are missing data but don't hint as to what data they would display. The dashboard widgets are actually completely customizable, meaning that (technical) users can configure the queries and data displayed in the widgets. But you're right, maybe some transparent grey'd out random data would be helpful for visualization. > - Last updated date is always Oct 10, 2018 That's a placeholder date, sorry about that. > - I realize the software is pre-release though I'm not sure what version would have pulled from the main branch at this time. I would love to have a searchable timeline approach to recording/retrieving medical data, but pretty much all home care is going to be manual entry and the current manual entry flow is unusable. Will that see a big jump in v1? Here's our current roadmap - https://docs.fastenhealth.com/roadmap Search functionality is one of the next things we'll be prioritizing. I'd be happy to iterate further on the manual entry wizard, but I'd need some ideas/guidance for how we can make things better there. I really do appreciate all the clear feedback you've provided. My goal has always been to build something that could automatically sync my medical records, but also allow people like my parents to enter their data manually - Fasten will need a lot of help to get there.
