Looking for a self hosted task history / weekly report tool

At work I have to write a report every week summarizing the work I did
on my projects and their current status. At the end of the year, it's
really nice to combine this all together and use it as the basis for
my annual review. The problem is that we put our reports into OneNote
(bleh), so it's just bolded project names with bullet lists
underneath. Combining it at the end of the year means a ton of
copy/pasting.

The workflow I envision for each week is:

* Create a report where I enter the tasks I did for a given week ie
datestamped
* Each task is associated with a particular project

At the end of the year, then I can get all of the tasks by project for
some kind of annual report

I don't need time tracking, todo lists, or task tracking. Hoping to
find something with a nice web ui that I can self host, otherwise I
think I'll just put it in excel.
