So for something more useful I have a (work in progress) Docker to get
the  missing albums from lidarr and then add them to metube... but
does  something like this already exist?

interesting, especially for impossible to find / stream stuff.

does it download as audio-only? is there a way of directing lidatube
downloads to my music libarary rather than the usual metube directory?

Yes it's mainly just for the hard to find stuff...

It adds (album) playlists to metube and tells it to download an mp3 in
the best quality.

No but it specifies the folder name as "Artist - Title"
