Attached is the form you need. This will probably be a requirement in the Re-Application process, therefore, take it to a council meeting and complete during the meeting. This will help with your IHI Contract deliverables and prep for the Re-Application. BJ Butler and myself met with Patsy Nelson and we believe that we can do a Re-Application for Coordination Funds for Comprehensive Community Health Improvement Councils (prenatal to seniors) instead of an RFP so long as we use current contractors. Either the CMCH or the IHI contractors from this year. We are in the early development stage of the Example Scope of Work and the Re-Application. What we can tell you is that we are going to request a Spring 04 Update of the Community Health Profile, Community Health Improvement Plan and the CHIP-SAT for the Re-Application. So while waiting for the Re-Application to be released you can start preparing some of these documents. For those counties that do not have IHI funding for such updates, an application was emailed today announcing funds available to do the updates. Here are some key talking points that were shared at the different health council leadership meetings regarding the Profile Updates. *The Community Health Profile and Plan is a Living Document or a Work in Progress. It is not a pass/fail document. *The “Blended Planning Document Guidelines for Community Health Improvement Councils” is the format all IHI funded councils shall follow for their minimum data sets. CMCH is encouraging this format to be used this year if the council is updating their planning products. (New Information: IHI funds will be available to the ten counties, not currently funded by IHI, to financially assist them in updating their profile and plan to use this format and to include adult and senior populations). *Use the “Blended Planning Document Guidelines for Community Health Improvement Councils” as the template/outline. If some recommended information is not available, the council just documents “information unavailable at this time” in the space as a placeholder. This assists DOH and the community in understanding what information is currently not available. Attached is the blended document that we are requesting the Updates be formatted into.
