Sam will *never* notice the difference: the implant she received at birth (as every American has since 1947) has been reprogrammed, so that she accepts this new Alex as the child to whom she gave birth. Since Luka, as a foreign-born individual, lacks this implant, he *knows* that Alex has been replaced.
