Published by DATAMOST, which I suspect is no longer around.  Randy Hyde
is though; not too long ago he had an internet address at a university
(UC Irvine?)  Perhaps somebody else can fill in the blank (send e-mail).

The Acknowledgements section is enlightening:

"This manual was written on an Apple II computer system using the Apple PIE
(PIE Writer) word processing system available from Hayden Book Company.  The
original illustrations and drawings were created on Apple's LISA machine
using LISADRAW.  Without these two computer systems this project would not
have been possible."

The book gets about as gritty as it gets.  It even has a section on how
to interface peripherals and non-standard devices to Apple Pascal.  Might
be hard to find though.
