     Just occurred to me that, maybe, you were talking about the GS/OS part of
GS System. (For a long time, everyone called GS System "GS/OS", including the
Apple II magazines which announced system releases.)

     If you are talking about the GS/OS part of System, I don't know that a
v4.0 was ever released.

     Scanning through the Start.GS.OS files for various releases of GS System,
you find the following:
