Learning Workshop, you may be intested in this additional information

1)  It would be nice if the first rule above the title were two inches from
the top, but if space is a problem it could be even just 1" from the top
and that would not be a problem.

2)  Indent it 1/4".

> 3) What is point size of footnotes?

3)  I believe the point size of the footnotes is approximately 8 or 9 points.
Either would be fine.

4)  The bottom margin should be no less than 1/2".

> 5) How big is the gap between columns?

5)  The space between columns is either 2 picas or 1/4".  Something closely
approximating that would be fine.
