Does the "DESC" file have the pagelength parameter right?  You'll want
to set it to 9 inches.  In order to not interfere with other uses of
ditroff, you'll probably want to copy the /usr/lib/font/dev<xxx> ASCII
width tables to /usr/lib/font/dev<yyy>, adjust pagelength, and recompile
the widths with makedev.  (or PD mkfont if your ditroff didn't come with
makedev).  This should solve (2), and make (1) irrelevant.
