The version of ditroff from ELAN has proven reliable.  I have
only used the nroff side of things extensively, but the files
move right over to our SoftQuad Troff on XENIX where they are
output on an Apple Laser Writer.  

Troff on a PC is going to be slow.  I would recommend an AT
class machine if you want to get work done quickly.  Troff
does quite a bit of thrashing around on the harddisk on my PC.

ELAN's software even allows inclusion of graphics in the raw
file.  This is sorely needed in troff.

I primarily use SoftQuad Publishing Software on a XENIX 286
box, and have found this product to be fantastic.  They have
enhanced troff with all kinds of goodies like long macro names
(up to 14 characters), debugging facilities, and other neat
things. Is XENIX out of the question for you?

BTW, they have an agreement with AT&T to handle production of
future incarnations of the Doc. Workbench, so it may not be a
bad way to go.
