Claim for a missing tooth

Use this service to claim financial reimbursement for a tooth which
has been lost and cannot be collected by the Tooth Fairy - for
example, teeth which have been:

  * accidentally swallowed
  * stolen by a ferret
  * destroyed by an asteroid
