Create a Form

Enter markup describing your customer form (see example below). Click
[create form] and you will be brought to a link with the online form.
Mail the link to your customers and they can then fill the form in
their browser and print it as on paper or as a PDF.

It's recommended that you edit your form markup in your favorite text
editor, notepad (or VI), and then paste it in here. This saves
heartache and despair when your browser muffs things.
