	Some of these updates have been overtaken by out-of-band
	contributions to revision 1.14.  I've added the rest.  In the
	case of Dvorak, I've used the country names specified in
	Baerenreiter's "Die Musik in Geschichte und Gegenwart".
	Czechoslovakia didn't exist then, and it no longer exists, so
	it's not an appropriate name to use.
