  I recompiled the kernel with the "device   pcm" option.  After installing and rebooting the kernel recognizes the "Creative SB 16 PCI PNP" card - but dmesg includes the following lines:

pcm0 <Creative CT5880-C> at device 9.0 pci0
unable to map register space
device_probe_and_attach: pcm0 attach returned 6

I did not try (this time) the ./MAKEDEV snd0 since the card was not initialized.

  I don't have a fix, but I plan to install another "Creative SB16 PCI PNP" card that was previously identified by FreeBSD 4.2 and Red Hat Linux as Ensonic 1571.  I'll pass along the results of the switch.

The reason for the quotes on the name of the sound cards is that they are visually identical and came from identical boxes, but FreeBsd and Linux identifies them as different cards.

 FIXED!  On a hunch, I unselected PNP OS in the BIOS (which I had selected to 
 keep the device probe from hanging during install and/or /stand/sysinstall 
 probes).  Not only does the sound card now work, but my PCI PNP modem is 
 recognized and moved to a non-existent serial port.  I did not install the 
 substitute sound card - too much work dwhen compared to configuring the BIOS.
 
 Case closed - but someone else might get bitten by the same dog!
