After that there is way to get da0 back other than reboot.
Rebooting with CD writer on is fatal also with similar messages.
On scanbus the list was much longer, of course. And it took some
time. And froze all disk operation until crash.

This appears to be a problem with the NCR driver.  Stefan wrote the NCR 
driver, and is the person most likely to be able to decipher the NCR 
error messages in this PR.

 Does the problem still exist for you?
 I have the same controller and rescanning works for me (but I have different disks)
