      With a NVIDIA GeForce MX card installed:
Activate TwinView option in XF86Config.
Put correct data about tv-out.
Start X.
If ererything goes ok, terminate X and repeat it from the previous step until the bug appears.

 Are you using the nvidia driver, or the nv driver?  You forgot to
 include your config file in the PR.
 
 Either way, this sounds like a bug in third party code (the nvidia
 driver or XFree86) that you should take up with them.
