Compaq Proliant 1850R with Smart Array 221 Controller with 2 disks configured
as one logical volume

>Description:
        Machines with ida(4) supported hard disk controllers will panic
        on boot in ida_construct_qcb() as GEOM starts inspecting the first
        disk on the ida controller. Panic does not occur with ida(4) driver
        disabled.

        Last message before panic on verbose boot is:
        GEOM: new disk idad0

        DDB trace at panic shows:
        ida_construct_qcb()
        ida_submit_buf()
        idad_strategy()
        g_disk_start()
        g_io_schedule_down()
        g_down_procbody()
        fork_exit()
        fork_trampoline()

        Panic is:
        Fatal trap 12: page fault while in kernel mode
        supervisor read, page not present
        pid is 4 (g_down)

        The panic occurs on ida_construct_qcb+0xca: movzbl 0x5c(%eax),%eax
        which looks very much like the spot where this function bzero()s
        the hardware qcb.

        This was previously reported on the current mailing list by
        Ventsislav Velkov <veno@evrocom.net> on 31 March 2003
        (Message-Id: <006401c2f776$d16f0230$23f00ad9@beatle>) on similar
        Compaq hardware. It is likely that this bug has existed
        at least since that time.

	(This is a correction to pending/53238 which was mangled by my
	mail client, sorry.)

>How-To-Repeat:
        Attempt to boot 5.1-RELEASE or 5.1-CURRENT SNAP's (either floppy or
        CD) on systems with ida(4) supported hardware with at least one
        volume configured on the ida controller.

	Unknown.

 Just a "me too" here. Running on a ProLiant 5500 with a Compaq Smart Array
 4200 (1.44 Rev. A), four disks as one logical volume.

 Tried both 5.1-RELEASE and recent snapshots.

 This also happens on two machines,

 HP Visualize X-550
 COMPAQ Professional Workstation

 Both are equipped with a SMART ARRAY/2-DH v. 4.50c firmware.

 The arrays are configured with 2 physical disks, in 4 logical
 drives.

 The panic happens after GEOM looks at all four logical devices.

 Please reopen PR:

 If 5.2 RE-team included this fix into 5.2 Release Installer (I think
 they've done this, because it was a stopper issue) the fix doesn't work
 on a Proliant 1600 (P08,System Rom 11/08/2000, SmartArray 221, rev. a,
 F/W 4.50, 3*9.1 GB Drives attached as Logical Raid-5 Volume).

 The installer (mini-inst-5.2 Release.iso) of 5.2 Release hangs with a
 kernel-panic right after "Waiting 15 secs for SCSI devices...".

 4.9 Installer works on the same box without trouble.
 4.9 dmesg available on request (5.2 unfortunately not - as I'm not able
 to install it with this problem).

 Little Addition:
 Same situation with the current JPSnap cdboot.iso
 (cdboot-5.2-CURRENT-20040121-JPSNAP.iso) - where the fix
 should be in, shouldn't it?

 JPSNAP kernel message:
 kernel: type 30 trap, code=0
 stopped at strcmp +0x4: movl 0x8 (%ebp), %edx

 Hi!

 I can confirm that the problem is still present in 5.2.1-RELEASE. We have=20
 the same problem on one such Compaq Proliant 1600R (P08, System Rom=20
 11/08/2000, SmartArray 221, rev A, Firmware 4.32, 2 x 18GB drives in 1=20
 Logical Volume (RAID1)).

 Booting (verbose) 5.1-RELEASE, 5.2-RELEASE and 5.2.1-RELEASE gives the=20
 exact same symptoms: right after the "Waiting 15 secs for SCSI devices to=20
 settle", right when the installer would (or does) start sysinstall, the=20
 system freezes. Starting in verbose mode shows that GEOM starts to look=20
 into the drives not long before the crash. I can reproduce the problem at=20
 will; simply booting off a 5.2.1-RELEASE miniinst will do it.

 Depending on which OS Type is selected (and APIC settings) in the setup, we =

 either get a freeze right after "launching sysinstall on vt0" or a panic=20
 similar to this: "panic: unknown/reserved trap" and the proceeds to flush=20
 buffers and never finishes. (more precise manual transcript can be=20
 provided). On the other hand, once the APIC setting is set tu "full-table"=20
 as found in the mailing list archives, it seems to no longer be a problem.=20
 I don't believe this issue is related to kern/53245 though.

 A complete 4.9 dmesg can also be made available upon request.

 Please, let me know what other information can be useful. We have 3 or 4=20
 other identical machines, just waiting to be converted to FreeBSD, and I'd=20
 rather wait and go with 5.x directly. Anything else we can do, short of=20
 physically sending the machine somewhere?

 Thank you!
 Antoine
