 Here's the output.  FWIW, I reset my BIOS defaults to "fail-safe",
 and tried turning off the parallel port and USB controllers to see
 if that made any difference (it didn't).  I haven't (recently) tried
 a different I/O card slot.  I have had success with an idential SIIG
 card a different machine (to which I no longer have access).  Let me
 know if there is anything else I can or should try.

 It looks like the fourth port failed some of sio(4)'s tests.
 I wanted to make sure that it had its resources assigned ok
 and it does.  Have you tested all 4 ports on this board under
 another OS?  Also, have you tried a different board of the same
 type under FreeBSD?  It may be that that port is just bad on
 that card.
