Then I tried to boot 4.6-RELEASE install disk on Intel Sitka platform with Mylex eXtreme RAID disk array (Sitka - 4xXeon, SC440NX board) it fall into kernel fault on mlx device detection.
Controller is used with latest firmware & BIOS (7.01-00/6.01-30)
It working fine without controller. (FreeBSD was used on this system more than 2 years)
Same situation is with installation floppy disks and with custom kernel.

Try to boot 4.6-RELEASE Install CD with Mylex eXtreme RAID 3000 controller installed with latest firmware.

 Guys, we already buy this hardware, relying on "support in FreeBSD".
 As soon as I 'll have time, I 'll try to resolve this problem myself, but
 now... $3k hardware just keep idle...
  
 I like FreeBSD, and was with it long time. All Nizhny Intel site Unix
 infrastructure is based on FreeBSD.
 But now... Lot of problems with last 4 releases... NIS, filesystems,
 hardware support...
 And, unfortunately, I can't promote it in Intel anymore - because of these
 problems...

I only have access to an eXtreme RAID 2000, and it appears to work fine. 
Can you try removing the card, installing onto another disk, inserting 
the card, and capturing the crash via a serial console?  Also, does it 
behave any different under 5.0-R?
