(The 'name slot allocation failed' messages happen during multi-user
operation, when modem_run loads the microcode to the modem.)

Sometimes (rarely) the system outputs
    usb0: host controller halted
repeatedly.

This always coincides with all lights off on the Alcatel modem, and
the IP connection being dead.

This state can only be remedied by a reboot. The modem lights come
back on only after the BIOS has gone through the reset phase.

If the system is not rebooted for an extended period of time, i.e.,
after quite a lot of the above error messages have been printed,
it reboots spontaneously (without a dump, although these
are enabled).

(Note: I have a suspicion that the Alcatel modem might be powered off
through a remote command from the CO. However, this is just a wild
guess. I would like to have a command to reset the USB
port while the system is running multi-user.)

There have been a lot of improvements to the usb stack since this 
report was made.  Can you please reconfirm whether the problem still 
exists in RELENG_4 as of today, or 4.9-RELEASE.

 I also get this error. Details are 
 
 System: 
 	Currently 4.9-RELEASE (cvsuped+buildworld etc. as of 4 days ago) but has been 
 occurring from when I had the modem, IIRC from version 4.4.  gone through 
 various hardware changes (including completely different box) upgrades, clean 
 installs etc. and always had the issue. 
 	pppoa-1.2b2,1 (installed from ports) though also have installed it from 
 source previously 
 	Running - ipfw + natd (system is used as gateway/firewall) 
 
 Details: 
 	Get 'usb0: Host Controller halted' message, and need to reboot to resolve. 
 Happens infrequently (maybe less than once a month) unless I am using a p2p 
 client, when it happens frequently. I can download from ftp/http etc. at full 
 speed without a problem, and can usually download from p2p client (guncleus/ 
 bittorrent) at low speeds without the error occurring, but when I start to 
 use a lot of the bandwidth  with the p2p client, it will almost always cause 
 the error. 
 
 Other: 
 	I would be willing to offer ssh access to my box to anyone looking into the 
 problem if it will help.
