unfortunately no longer have dump got overwritten from another page fault
for other problem

Likely a standard SU/bgfsck problem.

This looks more like a classic bgfsck/SU issue. Does this still happen
if boot into single user fsck and then try a clean shutdown?

 Apologies for late reply.
 
 there was no bg fsck running at the time reboot was issued, if I issue
 a shutdown from single user mode there is no crash.
