I usually start it with: ppp -alias -background ibm

When it does work to the point that I can PING the outside
world, I get the following message:

   ppp[241]: Error: Got ConfigReq while state = 4


I am also running Samba, which works fine, in case there
may be any coexistence problems.

Sounds like that ISP that drops the line leaving ppp in a  
STOPPED state :(  Ppp's mine.

 Can you try putting a "set stopped 5" line in your config file and 
 say if it helps ?
