Is there a reason this hasn't been committed? Do I need to make some changes?

I just looked through the patch and noticed a few things.

It references the send-pr webinterface which has been disabled (due to
abuse) so I don't think it is useful to add a link to it at this point.

In The FreeBSD Documentation Project word list 'e-mail' is spelled
 'email'.

The latest update to the 'Writing FreeBSD Problem Reports' article uses
'[maintainer update]' so for consitency I think it would be better tell
the maintainer to use that here also.

As to why it hasn't been committed / commented on I would say most
likely just because there is a backlog of PR's and nobody has gotten
 around to look at it.
