Have installed a 'basic' copy of 4.2 on a Gateway 5300 Laptop. dmesg reports the pccard items as follows;

pcic0:  <Intel i82365> at port 0x3e0 iomem 0xd0000 on isa0
pcic0:  Polling mode

The following PC Cards have all been tried;

Lucent Technologies WaveLAN IEEE
3Com EtherLink III 3C589C
NE2000 PCMCIA Ethernet Card

All three of these cards work in other laptops configured with FreeBSD 4.2

When starting pccardd with the -d option the following is displayed;

Files in etc/defaults are stock standard and have not been changed!
/etc/defaults/rc.conf has pc card support enabled.

Get a Gateway 5300 laptop, install FreeBSD 4.2 and enable pccard support, then insert any pccard.

This looks a lot like pilot error.  Have you tried a different pccard_mem 
address in your /etc/rc.conf file?
