>Description:
I was unable to boot the FreeBSD 6.0-RELEASE install CD 1. I got a message
saying 'panic: ohci_add_done: 0x0009fbd0 not found', followed by a reboot
after 15 seconds. Tried to load the kernel with option#7 (usb keyboard)
and no-acpi, but it didn't help.
>How-To-Repeat:
That's kind of easy, I only have to insert the CD and try to boot from it.

Removing the USB keyboard and using one which plugs to the PS/2 port
makes the problem go away, so I guess there is either something wrong
with my keyboard or with the ohci driver...

My system is a Dell Dimension C512 with nForce 430 chipset. It panics
with the USB keyboard plugged in during boot with 6.2-Beta Install CD.
Since there is no PS/2 port on this machine, I hope this problem could
 be fixed before 6.2 Release.

I have the same issue with a Dell Dimension E521 (Athlon 64 x2) with
 USB only. No PS/2 keyboard.

 Tried, 6.1 R on x86 and AMD64. And 6.2 Beta 2 on AMD64

 Some follow-ups here:

 1. Unplug the keyboard when it is booting, plug it back in when
sysinstall appears seems to be a work-around for the install CD in 6.2
 Beta.

 2. Upgradding to 6.2-PRE-RELEASE fixes the problem for usb keyboards.
The system no longer panics, but will panic if a usb mouse is plugged
 in during boot phase.
