Mostly sloooooooooowly. :D  Some Europeans have built high speed packet (in the
Mbps range of speeds), but it hasn't spread outside those areas.  There's also
wifi style mesh networks that are capable of fairly high speeds.

And recently, someone released an open source/open hardware "next generation
packet radio" implementation.
