I certainly use Synchronet as a sysop, though Rob (DM) is the logical person here. :) Synchronet is certainly one of the major players in today's BBS scene and has some really nice features. I personally make use of its flexibility in message handling. I use FTN, QWK and email (both to host mailing lists and to gate existing mailing lists into the BBS). I had some connections with the underground scene here, though I kept at arms length from it myself. One thing I do miss from the past is the diversity of message networks in the past. That diveristy now resides largely in places like Facebook and Reddit. BBSs no longer have the user base to support it. Sadly, the walled garden nature of the Internet makes it impossible to tape into this to diversify BBS messaging. Back in the heyday, alongside Fidonet, we were linked to networks covering ham radio (I do run a net for radio comms, but keep it broad), the occult, LGBT and Buddhism, among others. The BBS was shut down over 20 years ago, so I'm relying on hazy memories. :) ....
