Most of that information I expect to have been already entered into my on-going medical records, I do have a probably quarter-sheet with my prescriptions, dosages, who issued and why and another quarter-sheet listing doctors (including dentist). Phone has a scroll for preferred hospital, medical doctor, and contact info. Looking at your list it might be a good idea to add another sheet (might have to go half!) of medical history summary. ...The quarter-sheet size folds and fits in with the insurance cards - all together in my wallet.
