The good news is the various insurance companies I've dealt with (when employed, retired and private insurance, now Medicare) if they haven't paid there has been a valid reason: not meeting the deductible yet, improperly coded (was resubmitted and paid), or just not covered (my optometrist's office now likes to submit even though I'm not covered - OK, I just hang on to my mony a couple of months longer).
