Well there used to be the old kids' trick probably back in the 1960's 
where someone in (say) Boston could dial the Operator, ask for the Nome,
Alaska, Operator, the ask the Nome Operator to call a neighbour,  Must 
have been another step: person-to-person (?) as somehow the neighbour
had to be told they were being told they were being called by Alaska.  
My friends never did it as we figured we'd manage to get in deeeeep 
trouble!
