NCVEC605.PDF    Revised NCVEC Form 605, effective August 21, 2017. The
 new form asks "the felony question". See page 2 of the PDF file (the
 back of the form) for details. Requires the Adobe Acrobat Reader to view.
 Note that the NCVEC Form 605 must be dated September, 2017 or later to be
 acceped at a license exam session. Any earlier versions will invalidate
 the license exam session result and application for new or upgraded ham
 radio license, sequential callsign change, name, address, or email change,
 etc.
