Convert CP437 to UTF-8 (when necessary) for *nix stdio doors (e.g. DOSEMU)

.... as requested by "ben" in #synchronet at irc.synchro.net

Currently, this assumes all stdio doors aren't "UTF-8 native", which might be
an incorrect assumption.
