Probably. I would have skipped the Hold fase in this case. "Hold" is to be used when the condition is expected to be temporary. I doubt there has been any contact with te sysops in question. The expectation is not that they will be back on line in the foreseeable future.

Indeed. And this has been the case for a very long time. Might as well skip the formalities and remove it right away.
