Standard-wise an IPv4v6 (dual-stack) PDP context is a rather new feature which is hard to implement with existing roaming partners in mind. Basically these became available with 3GPP release 8 networks (3GPP LTE is also specified in release 8). So the devices would need to open two PDP contexts (with different APNs) for IPv4 and IPv6 to have dual stack. This of course doubles ressources in the network. I've seen it working in the mid-200x years on Nokia mobile phones. Other mobile vendors still had problems with IPv6 bearers in 2011. Even modern LTE devices (from some asian vendors) had problems with interoperability when you had IPv4v6 PDP contexts. Many baseband chip vendors asked extra license fees for IPv6 support ... so it will take another while until we see proper IPv6 in our 3GPP networks ... operators are rather cautious -- every support call reduces their margin.
