I. STAFF IDENTITY CARD Issuance Members of staff will be asked by the Personnel Office to provide two recent photographs of themselves and each of their dependent family member(s), if applicable, for the issuance of a Staff Identity Card and Staff Dependant Card(s) upon assumption of duty. Purposes The Staff Identity/Dependant Card will serve as a personal identification; a pass for entering the University premises; and a certification for using University facilities. For staff members and their dependent family members who have joined the New Medical and Dental Benefits Scheme offered by the University, the Staff Identity/Dependant Card will serve as an identification and certification of their eligibility for medical and dental benefits under the Scheme when visiting the appointed doctors and dentists. Staff Identity Card/Dependant Card is not transferrable to any other person for use. Loss Members of staff should report on loss to the Personnel Office at the earliest possible time. A fine will be charged for replacement of Staff Identity/Dependant Card. Staff Dependant Cards Staff Dependant Cards of dependent children should be returned to the Personnel Office when the children reach the age of 19 and cease to enjoy the benefits provided by the University, unless supporting documents are provided to show that they are still in full-time education. Staff members should return their Staff Identity Card/Dependant Card(s) to the Personnel Office on leaving the service of the University. .
