The moth is the symbol for Lunesta, the new sleeping pill launched earlier this year by Massachusetts-based Sepracor Inc. with a $60 million advertising campaign. Like Dorothy's wizard, the Lunesta moth has kept a few things close to the vest - including some Food and Drug Administration reviewers' initial concerns that the drug might pose a cancer risk. Although the FDA reconsidered the data and ultimately approved the sleep aid, a watchdog group still questioned why patients would choose Lunesta. And that question will prove crucial to Sepracor as the field gets more crowded with rivals. Lunesta's key selling points are that it is effective over long periods, is approved for long-term use and helps users sleep through the night with fewer periods of wakefulness than some other drugs. John LeCroy, an analyst for New York stockbroker Natexis Bleichroeder, said estimates are for Sepracor to sell about $240 million in Lunesta prescriptions this year. LeCroy said that in the past couple of weeks, sales, after having hit a plateau, began growing again, with a week-over-week increase from 60,000 to 66,000 prescriptions. Despite Lunesta's initial success, the drugmaker must protect its flanks as newer sleep aids come to market. Lunesta is still far from achieving blockbuster status, and before it gets there must battle for market share against Takeda Pharmaceuticals' Rozerem, which the FDA approved this summer, and Sanofi-Aventis' Ambien CR, a new version of market leader Ambien approved earlier this month. Pfizer and Merck also have new sleep aids in development that could be available by 2006 and 2008, respectively. Rozerem carries the advantage of being the first FDA-approved sleeping aid not required to carry a label warning about possible addiction. ``The concern is how many docs will write prescriptions for Rozerem instead of Lunesta,'' LeCroy said. Ambien CR, whose earlier version is the top-selling sleep aid, has the advantage of familiarity among patients and doctors. So any safety issues - which recently have been under the spotlight - could become marketing handicaps for a new drug. Before the FDA approved Lunesta last year, agency medical officers who reviewed it were concerned about human and animal test data they thought pointed to possible cancer risks in humans. The reviewers recommended that the FDA send Sepracor a letter saying Lunesta was not approvable. Some of the reviewers' concerns involved findings of tumors in lab animals and DNA damage that could lead to cancer development. But an FDA official, Dr. Robert Temple, said isolated cancer cases in clinical trials did not implicate Lunesta. Temple said Sepracor should be sent a letter stating that it is approvable, the FDA re-analyzed the data, and in December 2004, the agency approved Lunesta. ``I do not believe that an unreplicated weak finding should lead us to conclude that there is a human risk,'' Temple wrote in response to the cancer concerns expressed by the FDA reviewers. In a recent interview, one of those reviewers, Dr. Russell Katz, director of the FDA's neurology products division, said most of his fears had been allayed. ``I think there was some concern initially about a lot of different tumors in animals, including rats and mice,'' Katz said, noting that it's not uncommon to have disagreement before the FDA approves a drug. Sepracor officials did not return telephone calls seeking comments. But Dr. Sidney Wolfe, director of the nonprofit Public Citizen Health Research Group, a Washington, D.C., watchdog organization, said he remained apprehensive about possible side effects from Lunesta. ``Why give a drug that is a carcinogen in animals to people for something like this ?'' Wolfe said``If it were saving people from dying from cancer, then the risk might be worth it.'' Like other sleeping pills, Lunesta can cause side effects, including dizziness and difficulty with coordination. Some experts question why Lunesta is necessary at all, noting it is chemically similar to some other sleeping pills and doesn't offer much new for millions of sleep-deprived people across the United States. Dr. Brian Strom, chairman of the biostatistics and epidemiology department at the University of Pennsylvania School of Medicine, is concerned about expensive advertising campaigns promoting new drugs such as Lunesta before possible side effects have time to show up. ``I tell my patients, the drugs you see advertised on TV are the ones you don't want to use. Let other people use them first,'' he said. Lunesta and two older, competing sleeping pills, Ambien and Sonata, all work similarly. They are known as hypnotics and belong to the nonbenzodiazepine family of drugs. Strom says: ``There is nothing new about this drug. Why spend more money on a new drug when you don't know its side effects?''
