Caliper can talk about their geocoding capacity, www.caliper.com send questions to peter@caliper.com There is new capacity in the new version. Mapt can read in TIGER files etc. Caliper's fame is due to TransCad, a professional GIS which of course totally depends on the quality of the transportation grid, that is, street files. I think they have this problem worked. Peter is their customer person and very sharp. He is not a used software salesman, but a professional geographer that knows all the details. Please be careful with online "free" services . You may have no way to validate the results or access to how they validated the results. Just because someone gives you a long and lat does not mean it is worth anything. Also be careful of advice from vendors or folks who geocode businesses and not residential addresses. A totally different problem. I would like to suggest something else. When one does a study with geocoded case data, say cancer deaths in xyz county. It is a big deal to check out the chrematistics of those cancer records that geocoded and those that did not. I guess you would be checking for geographic bias. Also it is important it quantify other differences, like those records that geocoded to intersections and not address house numbers, or to census polygon centroids, etc. Most important its necessary to know when you have one and not the other, or vice versa. Another comment, you are far more than a "lowly epidemiologist" in my opinion you should be total advocate for the tools you need. Usually management has no clue about what GIS tool is best for what task. So you need to tell them, Usually it is best to have access to more than one. If you are to do the work then you need to be the one making the decisions about the tools you need. Not management or the IT shop. And the other idea, "free" is seldom really free.
