Subject: Graduate Appointee Insurance

For students who would like to add a dependent this term, please 
have them complete another insurance information form and send a copy 
of this to payroll as indicated on the form.  Another copy is for 
your files and there is one for the student.  The last two copies 
(Graduate School and Insurance Company) should be returned to me in
the Graduate School, AG-10.  If you need additional forms, please
let me know.
