I would think the table of contents would tell them that?

Were you thinking:
accident, Chernobyl, 5-34   ?

or

accident, Chernobyl
   cause of, 5-34    ?

or  ?


Either way, if the whole book is about the Chernobyl accident, that would
be kinda broad! This is that meta-topic thing again, isn't it!?  And 30
pages is a very big stretch for a page range!  :-)

What are the significant conceptual details in Chapter 2 relating to "how
it happened" that could be main  headings?  Like:
    cause of accident   
    <prior events>
    <names of any  people involved>
    <specific factors involved>

These main entries could cover all the important info in chapter 2, but
more to the point.
