if anyone knows where to get the eligibility form, say so, because we need
that and i can't find it anywhere on the usa cycling website.  they
changed stuff around since track nationals.

we ought to take care of this right quick since the registrars office is
a pain in the ass to deal with.  it will probably take two trips to get
this form completed.
