Today I mailed submission forms to all official current members of our chapter. The introductory letter should give you all the details you need about inclusion. If you wish to be included in the directory, fill out the form. Electronic submissions are being accepted. If you'd like, forward the form below with your responses to Martha Osgood at index@teleport.com. If you do not wish to be included in the directory, simply don't fill out the form. If you are not an official member of the PNW chapter of ASI, but want to receive a copy of the directory, _fill out the form_ and at the list of payments on the end of the form, mark only the number of copies you want to receive under "extra copies" (directories are $8 each). Directory Information Form Dear PNW / ASI Member; We are in the process of publishing a membership directory for PNW / ASI. Complimentary copies of our directory and an introductory letter about our organization will be sent to publishers and publishing organizations in the Pacific Northwest who use freelance indexers. This directory will also be available online through our web site. Because this marketing effort needs to be self-supporting, there will be a small fee ($8) for inclusion in the directory. Members listed in the directory will receive one complementary copy of the directory. Additional copies can be ordered for $8 each (send these to your clients, or distribute them at speaking engagements). NOTE: orders for additional copies must be submitted with this form. Unless otherwise indicated, your listing will also appear on the PNW / ASI web site. We are also offering business-card sized, black and white advertisements for an additional $20 fee. These ads will appear in the appropriate "specializations" section of the directory. To include an advertisement in the directory, please return a camera-ready print copy with your payment.
